TBRHSC Trauma Program is requesting the following data from the Facilitators of a TIIPS Kit Program:

This information can be collected with the TIIPS Attendance Sheet (Appendix B) and then submitted
via TIIPS Data Form (Appendix M) to: traumaprogram@tbh.net or filled out online at our website.

Reporting of data to TBRHSC Trauma Program is requested following the delivery of the TIIPS
Kit Program.

Data collection and reporting provides an accurate database for quality assurance and ongoing
support for the TIIPS Kit Program.
